{"id":3107,"date":"2023-11-18T06:02:36","date_gmt":"2023-11-18T06:02:36","guid":{"rendered":"http:\/\/drhimanshuyadav.com\/f-hb-concentration-at-fit-may-help-define-screening-interval\/"},"modified":"2023-11-18T06:02:36","modified_gmt":"2023-11-18T06:02:36","slug":"f-hb-concentration-at-fit-may-help-define-screening-interval","status":"publish","type":"post","link":"https:\/\/drhimanshuyadav.com\/blog\/f-hb-concentration-at-fit-may-help-define-screening-interval\/","title":{"rendered":"F-Hb concentration at FIT may help define screening interval"},"content":{"rendered":"<div id=\"article-content\">\n<h3 class=\"sectionTitle\">    Top line: <\/h3>\n<p>patient&#8217;s feces <a class=\"cl_ref_article_2085614\" href=\"https:\/\/emedicine.medscape.com\/article\/2085614-overview\" target=\"_blank\" rel=\"noopener\">hemoglobin concentration<\/a> (f-Hb) on a fecal immunochemical test (FIT) may help determine optimal screening intervals such as: <a class=\"cl_ref_article_2500006\" href=\"https:\/\/emedicine.medscape.com\/article\/2500006-overview\" target=\"_blank\" rel=\"noopener\">colorectal cancer<\/a> (CRC)<i>.<\/i> <\/p>\n<h3 class=\"sectionTitle\">    methodology: <\/h3>\n<ul>\n<li>\n<p>In an ongoing study, researchers analyzed the dose-response relationship between f-Hb concentrations and the presence of advanced neoplasms (i.e., advanced adenomas or CRC). <a class=\"cl_ref_article_1819350\" href=\"https:\/\/emedicine.medscape.com\/article\/1819350-overview\" target=\"_blank\" rel=\"noopener\">colonoscopy<\/a> Screening in Germany.<\/p>\n<\/li>\n<li>\n<p>The analysis evaluated data from 7,398 men and women aged 50 to 80 years, after excluding younger and older patients and patients with colorectal cancer or a history of colorectal cancer. <a class=\"cl_ref_article_179037\" href=\"https:\/\/emedicine.medscape.com\/article\/179037-overview\" target=\"_blank\" rel=\"noopener\">inflammatory bowel disease<\/a>and those with other factors for colonoscopy.<\/p>\n<\/li>\n<li>\n<p>Participants underwent quantitative FIT (FOB Gold; Sentinel Diagnostics) before screening colonoscopy and f-Hb concentrations (\u03bcg\/g feces): <1.7 (SENTiFIT detection limit, 270), from 1.7 <8, 8 to < 9, 9 to < 10, 10 to < 12, 12 to < 17, 17 to < 25 (positive FIT threshold), 25 to < 45, and \u2265 45.<\/p>\n<\/li>\n<li>\n<p>Subgroup analyzes were performed within f-Hb categories by gender and age.<\/p>\n<\/li>\n<li>\n<p>Adenomas larger than 1 cm, adenomas with a (tubulo)villous component, and high-grade dysplasia were considered advanced.<\/p>\n<\/li>\n<\/ul>\n<h3 class=\"sectionTitle\">    remove: <\/h3>\n<ul>\n<li>\n<p>Overall, 41% of participants had f-Hb concentrations less than 1.7 \u03bcg\/g in feces, and 81% had f-Hb concentrations less than 8 \u03bcg\/g.  10% had a positive test result with an f-Hb concentration of 17 \u00b5g\/g or higher.<\/p>\n<\/li>\n<li>\n<p>The prevalence of advanced neoplasms increased from 6% in people with f-Hb concentrations <1.7 \u03bcg\/g to 22% in the group with the highest FIT-negative results, and in the group with the highest FIT-positive results (\u226517 \u03bcg\/g). ) group increased up to 51%.  \/g).<\/p>\n<\/li>\n<li>\n<p>The overall diagnostic accuracy was 9.9% positive rate, 39% sensitivity and positive predictive value, and 93% specificity and negative predictive value at a cutoff of 17 \u00b5g\/g.<\/p>\n<\/li>\n<\/ul>\n<h3 class=\"sectionTitle\">    in fact: <\/h3>\n<p>The study results &#8220;indicate that people at the upper end of negative FIT values \u200b\u200bhave a higher risk of colorectal cancer than those at the lower end,&#8221; the authors wrote.  &#8220;Although it adds complexity, this suggests benefits from personalizing screening intervals.&#8221;<\/p>\n<h3 class=\"sectionTitle\">    sauce: <\/h3>\n<p>The study was led by Dr. Tobias Niedermayer from the German Cancer Research Center in Heidelberg. <a href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M23-1358\" target=\"_blank\" rel=\"noopener\">Published online<\/a> on November 7th <i>Annals of Internal Medicine.<\/i> <\/p>\n<h3 class=\"sectionTitle\">    Limitations: <\/h3>\n<p>Limitations include limited generalizability to other FIT brands and other populations, statistical power, and cross-sectional design.<\/p>\n<h3 class=\"sectionTitle\">    Disclosure: <\/h3>\n<p>This study was funded by grants from the German Research Council, the Federal Ministry of Education and Research, and German Cancer Aid.
